Before first day of school

If your immunization information is not in English, please provide the original documentation as well as an English translation of the document.

 

Immunizations:
 

  • Polio - 4 doses

  • DTaP - 5 doses

  • MMR - 2 doses

  • Hep B - 3 doses

  • Varicella - 1 dose
     

Tuberculin Test 


The Santa Clara County Public Health Department TB Risk Assessment for School Entry form will be required for school registration effective June 1, 2014 for all children enrolling in kindergarten or transferring, at any grade level, from outside of Santa Clara County.  This form must be completed by a licensed health professional in the US and returned to the child's school.

 

Health Exam

This is also called the Child Health and Disability Prevention (CHDP) program and is required for 1st grade.  It must be completed no earlier than: 6 months before beginning Kindergarten and no later than: 3 months after entering 1st grade.  (Stated differently: between March in the year your child entered Kindergarten and December of their 1st grade school year) We appreciate you turning in this Health Exam Form (English and Spanish) as soon as possible.

Immunizations

Proof of your child's immunizations to date are required prior to registration.  There is a new requirement of a Tdap booster for all students entering 7th grade.

Per SB 277, the law allows parents/guardians of children with medical conditions, to be exempt to vaccinations with a physician's written statement including: the physical condition or medical circumstances of the child, which vaccines are exempted, whether the medical exemption is permanent or temporary, and expiration date of exemption when applicable. Schools maintain an up-to-date list of pupils with exemptions, so they can be excluded quickly if an outbreak occurs.  Please contact Health Services at (650) 302-1449 or (650) 302-1448 for further information.
